Your main responsibilities
- Facilitating hospital discharge
- Carrying out assessments in different wards.
- Managing a busy caseload within an acute hospital setting.
- Working closely with health colleagues.
- Ensuring adults in hospital are discharged in a timely and appropriate manner.
- Undertaking section 2s and 5s.
- Attending multi-disciplinary meetings.
- Mental capacity assessments leading to best interest decisions.
- Advising patients and families who do not meet the eligibility criteria and redirect them to other agencies.
- Advocate for the rights and entitlements of individuals, ensuring they have access to the necessary support services.
- Maintain accurate, timely, and up-to-date records of assessments, interventions, and outcomes in compliance with organisational and legal requirements.
- Stay informed about relevant legislation, policies, and best practices in social work and hospital discharge processes.
