Your main responsibilities

  • Facilitating hospital discharge
  • Carrying out assessments in different wards.
  • Managing a busy caseload within an acute hospital setting.
  • Working closely with health colleagues.
  • Ensuring adults in hospital are discharged in a timely and appropriate manner.
  • Undertaking section 2s and 5s.
  • Attending multi-disciplinary meetings.
  • Mental capacity assessments leading to best interest decisions.
  • Advising patients and families who do not meet the eligibility criteria and redirect them to other agencies.
  • Advocate for the rights and entitlements of individuals, ensuring they have access to the necessary support services.
  • Maintain accurate, timely, and up-to-date records of assessments, interventions, and outcomes in compliance with organisational and legal requirements.
  • Stay informed about relevant legislation, policies, and best practices in social work and hospital discharge processes.
