We are seeking a qualified Band 5 Occupational Therapist to join the ICRAS Central and South Urgent Therapy Team in Liverpool. The role is focused on providing reablement services to patients in the community, conducting assessments, and delivering treatment plans to support patients’ recovery and independence. The position is open to both full-time and part-time applicants. Candidates must have a valid driver’s license, as travel within the community is required.

Key Responsibilities:
  • Community-Based Assessment: Conduct home and community assessments for patients requiring urgent therapy and reablement services.
  • Reablement & Rehabilitation: Develop and deliver individualized reablement plans to support patients in regaining their independence and improving their quality of life.
  • Patient & Family Support: Provide advice and education to patients, caregivers, and families regarding the therapy and reablement process.
  • Collaboration: Work closely with other healthcare professionals, including physiotherapists and social care teams, to ensure coordinated patient care.
  • Documentation: Maintain up-to-date and accurate patient records, ensuring that treatment plans and progress are clearly documented.
Essential Criteria:
  • Qualifications: HCPC-registered Occupational Therapist.
  • Experience Level: Experience in reablement, community assessment, and treatment.
  • Skills: Strong interpersonal skills and the ability to work independently in a community setting.
  • Driver’s License: Must have a valid UK driving license, as the role requires travel to patient homes and community locations.
  • Availability: Ability to start on 11th November 2024 and commit to the contract duration.
Working Conditions:
  • Flexible working hours available (full-time or part-time).
