Job Overview:We are seeking a Band 6 Occupational Therapist to join the Urgent Therapy Team in Central & South Liverpool. The role involves providing rapid assessment and intervention to prevent hospital admissions and facilitate timely discharges for patients with complex needs.

Key Responsibilities:

  • Deliver urgent occupational therapy interventions in community and hospital settings.

  • Conduct rapid assessments to support hospital avoidance and early discharge.

  • Work collaboratively with multidisciplinary teams, including physiotherapists and social care.

  • Develop individualized care plans to improve patient independence and well-being.

  • Maintain accurate clinical records in line with NHS guidelines.

Key Requirements:

  • HCPC-registered Occupational Therapist with Band 6 experience.

  • Experience in urgent care, hospital discharge, or community rehabilitation.

  • Strong clinical reasoning and decision-making skills.

  • Ability to work autonomously in a fast-paced environment.

  • Car driver with access to a vehicle for community visits (preferred).
