What You’ll Be Doing:

Join our team as a CPN and play a key role in improving the lives of working age adults. In this role, you will:

    • The Crisis Resolution Home Treatment Team practitioner is a pivotal member of the Mental Health and Learning Disabilities division, to work as a member of a multiprofessional mental health team whose main focus is to offer an alternative to an inpatient admission and provide intensive treatment as required within the community.
    • The CRHTT practitioner will work across a 24/7 rota, inclusive of Bank Holidays, weekends, nights and to meet the needs of people in crisis at all times.
    • To work to professional and regulatory body codes, standards and guidance at all times ensuring clinical practice is grounded in evidence based theoretical and practical knowledge.
    • Gatekeeper service for hospital admissions 7 days per week and join Mental Health Act Assessments
    • Accept referrals and provide further assessments for service users on the caseload and are proactive in making autonomous clinical decisions, with high risk user population.
    • To take responsibility for planning and the delivery of care to people requiring an acute and intensive response from this service.
    • Work in partnership with doctors, family members and other involved agencies, e.g., local authority, housing providers, to inform them of progress, potential risks and contingency strategies to deal with anticipated problems.

Requirements & Qualifications:

To be successful in this role, you will need:

  • a current and valid NMC registration and pin
  • experience working with working age adults in the community
  • strong communication and interpersonal skills
  • the ability to work independently and within a multidisciplinary team
  • a commitment to delivering high-quality patient care 
  • access to a car for community/home visits

Working Hours & Contract Duration:

This role offers a flexible working position requires availability from Monday - Friday with flexibility where possible. The contract is for 3 to 6 months with a strong possibility of extension based on performance and service needs.
