MENTAL HEALTH SUPPORT PRACTITIONER - ESSENTIAL CAR USER POST

My client has an exciting position as a Mental Health Support Practitioner to join our Adults Services team based in Liverpool. Joining us on a full time, permanent basis, you will receive a competitive salary of £22561.50 per annum, plus £50 per sleep. You will play a key role in supporting vulnerable individuals and promoting positive outcomes in their lives.

Working hours:

8am - 8am

3 day rolling rota (1 on 2 off)

Responsibilities as our Mental Health Support Practitioner:

Mental Health Support Practitioners provide practical and emotional support to residents accessing the service as determined by their individual needs identified through assessment and support planning. They provide housing related support for vulnerable people to enable them to work towards fully independent living.

The successful candidate will implement and/or facilitate activities to develop individual support pathways that address specific needs including educational, social, emotional, physical health, and positive mental well-being. They will help support residents in maintaining medication requirements to ensure personal well-being is sustained, alongside increasing overall engagement via daily support interactions. Assisting with the safe management of the building will also be part of the role, along with dealing appropriately with any occurring incidents, following all reporting requirements associated with the support offer at the service.

What we’re looking for in our Mental Health Support Practitioner:

Previous experience supporting clients with mental health issues - Essential.

Excellent communication skills; a calm, patient, and confident manner - Essential

Experience supporting vulnerable individuals through crisis or periods of change - Essential.

Experience of assessing the needs of vulnerable adults specifically with those who are at risk of homelessness - Desirable.

Experience of working collaboratively with other agencies both voluntary and statutory - Essential

Experience of case work and support planning, risk assessments and key work - Essential

Driver with your own vehicle and business insurance - Essential
