Outreach Family Support Worker

We are looking for a compassionate and proactive Outreach Family Support Worker to join our client's Early Help team, providing tailored support to children, young people, and their families. This is a rewarding opportunity to make a real difference by delivering strength-based interventions that empower families and create positive, lasting change.

In this role, you will act as the lead professional, conducting early help assessments, developing personalised support plans, and coordinating multi-agency responses to ensure a whole-family approach is taken to address identified needs. You will work closely with families, supporting them through challenges and helping them build resilience by accessing appropriate services and interventions.

Direct engagement with children and young people is a key aspect of the role, ensuring their voices are heard and their concerns are addressed. Through one-to-one work and structured interventions, you will help shape support plans that lead to meaningful improvements in their lives. Regular Team Around the Family meetings will be chaired to review progress, identify additional areas of support, and adapt plans where necessary.

The ideal candidate will have experience in early help, family support, and multi-agency collaboration. Strong communication and organisational skills are essential, as well as the ability to build trusting relationships with families and professionals alike. A passion for safeguarding and improving the well-being of children and young people is at the heart of this role.
