Outreach Family Support Worker

We are currently seeking compassionate and motivated Outreach Family Support Workers to support children and families through early help interventions.

Key Responsibilities:

  • Complete Early Help Assessments with children and families
  • Develop tailored support plans based on identified needs
  • Chair Team Around the Family (TAF) meetings to review progress
  • Work directly with children and young people to explore their views and worries
  • Coordinate multi-agency support to ensure a whole-family approach
  • Support families to make positive changes and improve long-term outcomes

What We're Looking For:

  • Minimum Level 3 qualification in Childcare, Social Work, Social Care, Counselling, Youth Work or Education
  • Experience working with children, young people, and families
  • Strong understanding of early help and strength-based approaches
  • Ability to lead assessments and coordinate support plans
  • Confident in chairing meetings and engaging multi-agency partners
  • Good communication, empathy, and relationship-building skills
  • Passionate about making a positive difference in family lives
