Outreach Family Support Worker
We are currently seeking compassionate and motivated Outreach Family Support Workers to support children and families through early help interventions.
Key Responsibilities:
- Complete Early Help Assessments with children and families
- Develop tailored support plans based on identified needs
- Chair Team Around the Family (TAF) meetings to review progress
- Work directly with children and young people to explore their views and worries
- Coordinate multi-agency support to ensure a whole-family approach
- Support families to make positive changes and improve long-term outcomes
What We're Looking For:
- Minimum Level 3 qualification in Childcare, Social Work, Social Care, Counselling, Youth Work or Education
- Experience working with children, young people, and families
- Strong understanding of early help and strength-based approaches
- Ability to lead assessments and coordinate support plans
- Confident in chairing meetings and engaging multi-agency partners
- Good communication, empathy, and relationship-building skills
- Passionate about making a positive difference in family lives
