Qualified Social Worker, Later Life TeamPay rate £35 per hourContract role Vitalis are recruiting for a Qualified Social Worker to work in a Later Life Team based in St Helens.The Later Life service in St Helens works with individuals aged 67+ with age related frailty and mental health issues such as dementia. The team is a large team consisting of 15 Social Workers and 15 Assessment and review workers. This is a busy team with the Social workers completing Care Act assessments on individuals, devising care plans and reviewing services. The social workers have a busy caseload and are expected to manage a high number of referrals.
