Sefton Council are seeking 3 dedicated and experienced Social Workers to join their Learning Disability & Autism Team within Adult Social Care. This role offers the opportunity to work closely with young people transitioning into adult services, ensuring timely assessments and support during this critical period. You will play a vital role in managing complex cases, contributing to a supportive and efficient service. This is an exciting opportunity for experienced practitioners looking to make a difference in the lives of individuals with learning disabilities and autism.

Responsibilities:
As part of the Learning Disability & Autism Team, your key responsibilities will include:

  • Transition Management: Overseeing the current transition allocation list, ensuring timely assessments and smooth transitions for young people entering adult services.
  • Case Management: Managing a caseload of complex cases, providing assessments, and managing risks effectively.
  • Timely Interventions: Addressing any backlog of referrals and ensuring that assessments are carried out in a timely and efficient manner.
  • Collaboration: Working closely with other professionals, families, and carers to develop person-centred care plans and ensure holistic support for individuals.
  • Teaching and Support: Supporting the teaching and assessment of social work students during their placements and offering guidance to less experienced staff members.
  • Court Proceedings: Providing evidence in court as required, particularly in relation to care proceedings.

Requirements:

  • Have substantial experience in adult social care, particularly within learning disabilities and autism services.
  • Hold a degree in Social Work and are registered with Social Work England.
  • Enhanced DBS.
  • Access to own car.
