Liverpool City Council is seeking a dedicated and experienced Adults Social Worker to join our team. This role is critical in providing essential support to adults transitioning from hospital to home, ensuring they receive the highest quality care and reablement services. You'll work within a collaborative, supportive team committed to making a positive impact on our community.

Responsibilities
  • Hospital Discharge: Manage cases involving hospital discharges, coordinating closely with medical professionals to develop smooth, efficient transitions for individuals moving back home.
  • Reablement Services: Deliver person-centered reablement plans, helping adults regain independence and maintain dignity in their own homes.
  • Assessment and Planning: Conduct assessments to identify individual needs and develop tailored support plans, aligning with the latest best practices in adult social care.
  • Multi-Agency Collaboration: Work effectively with healthcare providers, community organisations, and families to provide comprehensive, integrated care.
  • Safeguarding: Ensure all safeguarding principles are adhered to, prioritising the well-being and safety of each person under your care.
Requirements
  • Experience: Prior experience in hospital discharge and reablement services is essential.
  • Qualifications: Degree in Social Work and current registration with Social Work England.
  • Skills: Strong assessment, planning, and communication skills, with an ability to manage complex cases independently.
  • Knowledge: Familiarity with current legislation, policies, and best practices related to adult social care.
  • Personal Qualities: Compassionate, resilient, and proactive, with a commitment to empowering adults in the community.
