A new and exciting locum opportunity has arisen within a fast paced Hospital discharge team in the Knowsley, Merseyside area.

The role will be based within a fast paced Hospital Discharge Team. The team is made up of a team of dedicated health and social care staff, working together to provide holistic assessment and support plans to ensure the best possible outcomes..

Responsibilities:
  • You will carry out assessments under the Care Act 2014 to determine eligibility need
  • Maximise rehabilitation services such as reablement & intermediate care where appropriate
  • Provide assessment and appropriate interventions to aid patients in achieving optimum recovery/rehabilitation and quality of life.
  • Work with patients and families addressing the impact of illness and treatment
To be considered for the Social Worker role:
  • 1-3 years experience of working within a similar setting
  • Diploma/Degree in Social Work
  • SW England registration
  • Experience of working under the care act and mental capacity act
  • Hospital experience is an advantage
