Service Care Solutions are seeking a dedicated and compassionate Social Care Assessor to join an Adult Services & Health team at Liverpool City Council. This is a vital role, supporting individuals to access the services and care they need to live independently and with dignity. You will be part of a professional team that is focused on delivering high-quality services to adults in our community.

  • £22.00 LTD per hour inclusive of holiday pay
  • 35 hours per week
  • 3 month initial contract with possibility of extension after this

Responsibilities

  • Conduct assessments to identify individual care needs and ensure appropriate support is provided.
  • Work collaboratively with service users, families, and multi-agency teams to develop care plans.
  • Monitor and review care plans to ensure they remain effective and responsive to changing needs.
  • Provide information, advice, and guidance on available services and resources.
  • Maintain accurate and timely records in accordance with organisational policies and procedures.
  • Ensure compliance with relevant legislation and Liverpool City Council standards.
Requirements
  • Previous experience in a social care or support role is essential.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with service users and stakeholders.
  • Strong organisational skills and the ability to manage a varied workload effectively.
  • Knowledge of relevant social care legislation and safeguarding principles.
  • Commitment to promoting equality, diversity, and inclusion.
  • Flexible and adaptable, with the ability to work independently and as part of a team.
