We are seeking a dedicated and professional Social Care Assessor to join an established Adult Services & Health team at Liverpool City Council. This is a critical role, supporting adults in need of social care. If you are passionate about making a difference and thrive in a fast-paced, collaborative environment, we want to hear from you.

  • 35 hours per week (Monday to Friday, 9:00 AM - 5:30 PM)
  • 1 month initial contract with possibility of extension after this
  • 21.00 LTD per hour inclusive of holiday pay

Responsibilities:

  • Assess the care and support needs of adults and their families, ensuring compliance with statutory frameworks.
  • Work collaboratively with health professionals and other agencies to develop and implement care plans tailored to individual needs.
  • Provide guidance to clients on accessing appropriate support and services.
  • Maintain accurate and up-to-date records in line with council policies and procedures.
  • Ensure all actions are in line with safeguarding principles and promote the independence and well-being of clients.

Requirements:

  • Previous experience in a social care role, particularly within adult services.
  • Strong understanding of relevant legislation, policies, and procedures in social care.
  • Excellent communication and interpersonal skills to work effectively with clients, families, and multi-disciplinary teams.
  • Ability to manage a diverse and demanding workload, prioritising tasks efficiently.
  • A commitment to promoting equality and diversity in service delivery.
