The role: Social Care Assessor - Adult Social Care Team

Location: Liverpool

Rate of Pay: 22 Per Hour

Are you an experienced Social Care Assessor with a strong understanding of legislation and a practical approach to supporting fast-paced Adult Social Care Teams?

We are seeking a Social Care Assessor to join the Adult Access Team based at the Cunard Building. This is a secondment cover role, offering a unique opportunity to make a meaningful impact by working with adults aged 18 and over.

Key Responsibilities:

  • Conducting detailed Reablement Assessments and support plans.
  • Carrying out Care Act Assessments and Mental Capacity Assessments.
  • Supporting adults with varied and complex needs, including hoarding, substance misuse, mental health, older people, and learning disabilities.
  • Assessing eligibility for ongoing community support and providing reablement services to promote independence.
  • Working proactively to prevent, reduce, and delay the need for care and support.

Essential Skills:

  • Proficiency in Liquid Logic case management software.
  • Ability to manage a varied caseload efficiently in a high-pressure environment.
  • Strong knowledge of Adult Social Care legislation and best practices.
  • Excellent communication and assessment skills, with a focus on creating detailed, person-centered plans.
