This local authority based in the Merseyside area is seeking an experienced Social Worker to join the Adults Community Team. The Adult Social Care service has strong social work values and a focus on improving outcomes to vulnerable adults and their carers.

About the Role

We are looking for dedicated Social Workers to join our Adult Community Team. In this role, you will:

  • Conduct thorough Social Care Needs Assessments to identify and address the needs of adults in the community.
  • Develop and implement effective support plans to enhance well-being and independence.
  • Coordinate and manage services tailored to individual needs.
  • Assess and manage risks, including attending Multi-Agency Risk Assessment Meetings
  • Safeguard vulnerable adults and apply the principles of the Mental Capacity Act to support decision-making.
  • Manage legal aspects concerning adult protection, including Court of Protection cases.
  • Ensure compliance with Deprivation of Liberty Safeguards and utilize the Decision Support Tool for Continuing Healthcare assessments.
  • Work contracted hours within agreed rota patterns, including evenings and weekends as required.
