What you will do:

  • Communicate skilfully and confidently in complex or high-risk situations. You will also model effective communication skills and help colleagues refine their abilities in this area.
  • Represent the department at a variety of multi-disciplinary forums, and take on the role of Chair as required, showcasing leadership and organizational capabilities.
  • Undertake direct work with service users, carers, and families, following interagency safeguarding adults policy and procedures to ensure the highest level of care and protection.
  • Make appropriate assessments and act as a care navigator for individuals presenting eligible social care needs, collaborating closely with health and other agencies within specified time limits.
  • Depending on team requirements, work within a seven-day service or 8 am to 8 pm schedule to deliver integrated services alongside health professionals under the Care Act 2014.
