What you will do:
- Manage and effectively oversee a complex and diverse caseload, ensuring adherence to policies, procedures, and legislation.
- Prepare detailed reports, statements, and court representations when necessary, ensuring accurate and timely documentation.
- Employ professional judgement and risk management skills to make informed decisions on caseloads.
- Represent the department in various multi-disciplinary forums and, when needed, act as Chair.
- Engage in direct work with service users, carers, and families, adhering to inter-agency safeguarding adults’ policies and procedures.
- Conduct timely assessments and serve as a care navigator for individuals presenting with social care needs, collaborating with health and other agencies.
- Collaborate with relevant parties to design and deliver support plans that address individual needs within available resources.
- Actively participate in identifying service gaps and advising on improvements to service delivery standards.
- Undertake safeguarding enquiries and ensure timely and shared outcomes.
- Work closely with Social Care Assessors and hold the Approve Mental Health Professional qualification to conduct specialist duties, participating in the Out of Hours AMHP rota as agreed.
- Mentor and educate others, facilitating student placements and contributing to practice expectations.
