About the Role:Our client is a leading Adult Social Care provider is seeking three skilled Social Workers to join their Learning Disability & Autism Team. In this role, you will help manage the current Transition allocation list, address any backlog of referrals, and ensure timely assessments to support young people transitioning into Adult Services.This is an exciting opportunity to contribute to a service dedicated to improving the quality of life for individuals with learning disabilities and autism by ensuring a smooth and supportive transition into adult care.

Key Responsibilities:* Manage the current Transition allocation list for young people moving into Adult Services.* Address any backlog of referrals to ensure efficient service delivery.* Conduct timely assessments and reviews for individuals in transition.* Collaborate with families, carers, and other professionals to ensure a person-centred approach.* Support service users to achieve positive outcomes as they transition into Adult Services.

Working Model:Our client follows a hybrid working model, allowing flexibility between office-based work and remote working. An average week may involve a mix of working from home and in the office, ensuring a balance between collaborative team efforts and independent tasks.
