What You'll Do:
- Support individuals in their own homes, fostering independence and community inclusion.
- Work with people with a variety of complex needs (including Autism, Learning Disabilities, Mental Health, and Epilepsy).
- Provide person-centered care including personal care, medication management, and health & social support.
