Key responsibilities
The post holder will be part of the Islington Reablement Service - In-house provider services.
Reablement enables individuals to regain or maximise their independence through intensive, short-term support, building on their strengths and enabling people to live healthy independent lives for as long as possible in their own homes, or the place they call home. The Islington Reablement Service is for adults with a range of needs including learning disabilities, physical and sensory needs, mental health and older people both in community and hospital settings across various locations.
The service is currently delivered 365 days from 8am - 10pm. However, the future direction is for the services to become increasingly multi agency and the service may need to change in line with service development requirements.
The post holder will deliver personal care and support to people in accordance with their personalised support/Reablement plans. This may include personal care tasks, domestic tasks or exercise programs, developed by therapists to support people to achieve agreed outcomes and add to their rehabilitation or recovery at home. Care and support are given in an enabling way and the focus is on enablement and the promotion of independence. The Enabler will work closely with health and social care professionals, who are involved in the care planning and delivery of Reablement care. The post holder will report directly to the Reablement Senior Enabler/Coordinator.
Experience
- Experience of working for Adult Social Care or an equivalent agency providing domiciliary personal care.
- Experience of working with adults within in a Reablement setting is preferred
- Experience of working with people with differing needs and from black and minority ethnic communities in inner city areas.
- Experience and understanding of strengths-based approaches and applying this in practice
