About the Role
Haringey's newly formed Transitions Service is looking for a dynamic Team Manager to oversee a team of social workers and other professionals supporting young people transitioning from children's to adult services. This role involves managing Care Act assessments, ensuring robust case management, attending EHCP reviews, and fostering partnerships with professionals across education, health, and housing services. You'll work closely with young people and their families to ensure smooth, effective transitions, contributing to the development of this vital service.

Key Responsibilities:

  • Manage a team to deliver high-quality care and support for young people.
  • Oversee Care Act assessments and ensure attendance at key transitions meetings.
  • Support and coordinate with parent carers and external partners to ensure a seamless transition.
  • Lead and inspire a collaborative, high-performing team committed to the service's goals.

About You
We are seeking a qualified social worker (CQSW/CSS or DipSW) with current registration with Social Work England (SWE) and an enhanced DBS. You will bring extensive experience in social care, ideally within a local authority, and a deep understanding of the complexities involved in supporting young people with multiple service needs.

You will possess excellent leadership skills, demonstrated through previous experience managing or supervising staff, and the ability to motivate and foster a strong team spirit. Your strong communication, organisational, and problem-solving skills will be essential in navigating a fast-paced environment and managing complex cases. A commitment to the Haringey Vision and Values for Adult Social Care is essential, and you will have a proven track record of managing performance and meeting development needs.
