The Clinical Home Manager is responsible for the management and coordination of clinical care services for patients receiving care in their homes. Key Responsibilities:

  • Oversee the day-to-day clinical operations of home healthcare services.
  • Supervise and manage a team of healthcare professionals, including nurses, therapists, and support staff.
  • Develop, implement, and review care plans tailored to individual patient needs.
  • Conduct regular patient assessments and ensure the delivery of safe, effective care.
  • Ensure all services comply with healthcare regulations and industry standards.
  • Monitor patient progress, adjusting care plans as necessary and addressing concerns or issues.
  • Provide clinical leadership, mentoring, and support to the home care team.
  • Collaborate with physicians, family members, and other healthcare professionals to ensure holistic patient care.
  • Address any patient or family complaints or concerns, ensuring appropriate resolution.
  • Maintain accurate and up-to-date patient records and documentation in compliance with healthcare laws.
  • Conduct regular training sessions for staff to ensure the highest level of care.
  • Manage clinical budgets and resources effectively, optimizing care delivery while maintaining cost efficiency.
  • Stay current on industry trends, new treatments, and best practices in home healthcare.

Qualifications:

  • Registered Nurse (RGN) or licensed healthcare professional with clinical experience.
  • Minimum of 5 years of experience in healthcare, with at least 2 years in a clinical leadership or managerial role.
  • Excellent communication, leadership, and interpersonal skills.
  • Strong problem-solving abilities and the ability to work under pressure.
  • Ability to work independently and as part of a multidisciplinary team.
  • CPR/First Aid certification (or equivalent) preferred.
