As a Social Worker in the Independence and Access Team, you will be committed to supporting adults in promoting their independence, ensuring access to necessary services, and enhancing their overall quality of life. Your primary responsibilities will include conducting comprehensive assessments in accordance with the Care Act 2014 and other relevant legislation, focusing on identifying and addressing the specific needs of vulnerable adults. You will work to empower individuals by ensuring they have access to the right resources, services, and opportunities that enable them to live as independently as possible.



Responsibilities include:

  • Perform assessments according to the Mental Capacity Act.
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005.
  • To undertake person centred Assessments and Reviews for Haringey residents and service users and their carers in line with the Care Act 2014
  • Examine and supervise carer strategies.
  • Responsible for managing a caseload, conducting robust assessments in partnership with service users, their carers and multi-disciplinary partners.
  • Work closely with the Team Manager with a strengths based approach to provide personalised services with a clear focus on quality outcomes for our residents.



Requirements:

  • 2 years experience post qualification
  • Action safeguarding procedures
