My client in Southwark are seeking an experienced Assistant Practitioner. Suitable candidates must have prior experience completing Care Act Assessments or your CV cant be shortlisted. The main duties of the role include goal setting, completing Care Act assessments and devising care and support plans, completing reviews, , crisis intervention, signposting, providing advice and information and working with people with complex social needs. There is also a Safeguarding function within the service, which includes completing initial fact finding and assessing and minimising risk. Our Intermediate Care Service (ICS) is seeking an Assistant Practitioner to work within Reablement .
