Assistant Practitioner

Southwark Council are looking for an Assistant Practitioner to join their Older People and Physical Disabilities front door contact team, who are currently undertaking a transformation project which involves addressing an existing backlog of front door screenings.

Responsibilities:

  • Be responsible for completing screening assessments at the front door to Adult Social Care to determine whether or not an individual is likely eligible for services
  • Undertaking proportionate early intervention sign posting, support planning and proactively working with complex issues, family dynamics, legal issues, crisis, providing information and advice, transferring to another team if required and the provision of some ongoing safeguarding work
  • Arrange a telephone screening with the person identified to have potential need and/or their designated person, complete a screening of their mobility, transfer and self care needs and using the information gathering make a determination regarding basic equipment needs, order the required equipment from the designated equipment provider and signpost to other teams or services as required

Requirements

  • Experience of supporting adults with limitations caused by physical and mental illness and the ability to assess social care needs and initiate support plans
  • Working knowledge and understanding of equipment and adaptations and how this is applied to service delivery
  • Ability to undertake comprehensive assessments
  • Knowledge of rehabilitation, adaptations or equipment to assist disabled people
