Nurse Manager Job Description

Management

  • Ensure the Nursing Team work effectively to provide a high quality service to patients and to work effectively with other agencies
  • Develop, review and implement nursing protocols in conjunction with the Partners and Management Team
  • Support all members of the nursing/HCA team, providing guidance when necessary, conducting appraisals and training needs analyses as required
  • Co-ordinate the nursing team rota, ensuring sufficient staff are available to meet patient needs and hit the targets
  • Supervise and assess the Nursing Teams performance, ensuring a high standard of performance and personal development.
  • Identify gaps within the Nursing Team skill set, and ensure staff are upskilled and trained accordingly. This includes: Nursing Team, HCAs and GPAs.
  • Ensure resilience within the nursing team, in case of emergencies.
  • Support the recruitment, selection and retention of nursing staff to meet the health and well-being needs of the practice population
  • To organise monthly Nurse Meetings and follow up with any necessary actions
  • Offer and provide clinical supervision to other members of the practice nursing team and act as a Line Manager to HCAs, Nurses and GPAs.
  • Develop a working environment that promotes healthy, safe and effective work outcomes

Clinical

  • Identify, assess and manage patients with complex health needs, in all cases in accordance with practice policies & protocols
  • Continually review clinical practices, responding to national policies and initiatives where appropriate
  • To manage acute and chronic conditions and ensure the appropriate maintenance and use of chronic disease registers
  • To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Participate in local initiatives to enhance service delivery and patient care
  • To develop, implement and embed health promotion and well-being programmes and improve the health and well-being of the specific needs of communities, groups and individuals
  • To co-ordinate the provision of travel medicine services, and to ensure we promotion of the service
  • To support the team in dealing with clinical emergencies
  • Lead the child immunisation clinic and work with the nursing team to ensure a robust recalling system for 8 weeks checks and baby immunisations.
  • Identifying health trends and public health issues and working with the team and others to combat these by ensuring patient education and/or specific clinics (e.g. MMR clinics following the rise of Measles)
  • Working with public health initiatives ensuring the work of the team is up to date

Nursing Targets

  • Continually monitor the QOF targets and PIP reports to ensure we are on target and feedback to the Practice Manager and Targets Lead frequently.
  • Highlight areas where we are not on target and communicate with the Recalls Lead/GPAs/Management Team
  • Organise Flu, Shingles, Pneumonia, Health Check Clinics etc. to ensure we are profitable and providing excellent patient care
  • Ensure we have a comprehensive cervical screening recall system, and that smear results/colposcopy referrals are processed effectively and in a timely manner.
  • Leading the team in achieving the immunisation guidelines and targets

Compliance

  • Lead by example in promoting high infection prevention & control (IPC) standards in clinical treatment and throughout the practice to ensure a quality service. Ensuring all rooms are CQC compliant
  • Assist in compilation and implementation of policy, guidance and procedures relating to IPC in the practice
  • To maintain updated knowledge and skills in IPC and feeding back / share information appropriately on matters relating to IPC to all practice staff.
  • Participate in the review of significant events and support and participate in shared learning within the practice
  • Ensure all PGDs are kept up-to-date and that all nursing staff are compliant with them
  • Ensuring all rooms remain CQC compliant
  • Monitor stocking levels and ensuring no out of date equipment or stock in found within the rooms

Quality and Administrative

  • Work with the GPAs and Admin Team to reduce DNAs
  • Ensure all workflow and tasks are actioned in a timely manner
  • Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance
  • Work effectively with individuals in other agencies to meet patients needs
  • Effectively manage own time, workload and resources.
  • Any other duties commensurate with the role to ensure the smooth running of the Practice.
