The Role: Adult Mental Health Case Officer

About the Role:

We are seeking a dedicated Case Management Officer to join our Community Services Directorate within the Adult Social Care section. This role is instrumental in supporting working-age adults with mental health challenges, facilitating their choices and improving their quality of life through tailored care plans.

Key Responsibilities:

  • Act as the main point of contact for service users and their unpaid carers, ensuring a seamless integration of services.
  • Carry out comprehensive assessments and reassessments of needs, focusing on strength-based approaches.
  • Develop personalized support plans in collaboration with users, carers, and health professionals.
  • Manage caseloads efficiently, ensuring timely discharges and high-quality community-based care packages.
  • Promote self-directed support and direct payments, maximizing independence for service users.
  • Engage with a variety of stakeholders including health, housing, and community services to enhance the well-being of vulnerable adults.

Requirements:

  • Extensive experience in health or social care settings, particularly with mental health service users.
  • Knowledge of support planning, mental health impacts, and relevant legislation such as the Care Act 2014.
  • Strong interpersonal skills to co-produce care plans and manage risks.
  • Proficient IT skills across various systems.
  • Commitment to implementing equal opportunities and upholding the dignity of service users.
