About the Role:
We are looking for a dedicated EHCP Coordinator to join our SEND Service, ensuring legally compliant, child-centred, and outcome-focused casework. In this role, you will coordinate **Education, Health ensuring that children, young people, and their families receive the support they need. You will play a key role in coordinating assessments, casework, and multi-agency collaboration, ensuring that the process remains proactive, timely, and responsive to individual needs.
Key Responsibilities:- Coordinate assessments and casework across the SEND Service, ensuring legal compliance while prioritizing the needs of children and young people.
- Facilitate a person-centred, multi-agency approach to assessment, planning, and support, working in partnership with children, young people, parents, schools, and professionals.
- Guide schools and parents toward the right provision at SEN Support level and direct them to the Local Offer for further information on available services.
- Support settings and parents in providing the correct information when submitting an EHC needs assessment request.
- Manage and coordinate the 20-week EHC process, ensuring effective liaison with settings, parents, and professionals.
- Attend EHC plan review meetings, especially at key transition points, ensuring plans remain appropriate and effective.
- Chair and attend annual reviews for complex cases or where there is a high risk of placement breakdown, with support from Team Leaders or the Team Manager when needed.
- Act as the main point of contact for SEN casework-related communications, ensuring that all interactions with families, schools, and other agencies are proactive, supportive, and constructive.
We are looking for an individual with:
- Experience in coordinating SEND casework and the EHC assessment process.
- Strong knowledge of SEND legislation, including the Children and Families Act 2014 and the SEND Code of Practice
- Excellent communication and interpersonal skills, with the ability to build relationships with children, parents, schools, and professionals.
