As a Complex Care Mobilisation Nurse, you will be pivotal in conducting initial assessments and creating personalised care plans tailored to the unique needs of each client. Working alongside a collaborative team of nurse case managers, you will oversee the on-boarding of new packages, ensuring the highest standard of care is delivered.
Criteria for a Complex Care Mobilisation Nurse:
- RGN/RSCN with an active NMC PIN
- Proven experience in assessing clients with complex care needs
- Expertise in formulating individualised care plans
- Recent hands-on experience in tracheostomy care and ventilator management (within the last 2 years)
- Exceptional verbal and written communication skills for liaising effectively with patients, families, and the multidisciplinary team (MDT)
- Full UK Driving Licence
- Develop personalised care plans based on each clients specific needs
- Lead and coordinate the MDT, ensuring clear communication and effective teamwork
- Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
- Deliver nursing interventions, including the management of medication administration
- Collaborate closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
