Job Overview:
We are seeking experienced Community Nurses to join CLCH's dedicated team. In this role, you'll provide quality nursing care within the community, supporting patients at home while collaborating with multidisciplinary teams to enhance health and well-being.
Responsibilities:
- Conduct patient assessments and create tailored care plans.
- Perform nursing interventions, including wound care, catheter care, and medication administration.
- Monitor patient progress, revising care plans when necessary.
- Work closely with patients, families, and healthcare professionals to deliver holistic care.
- Ensure accurate and timely documentation in line with NHS standards.
Requirements:
- NMC-registered General Nurse (RGN).
- Proven community nursing experience.
- Competence in clinical tasks such as wound care and venepuncture.
- Strong communication and interpersonal skills.
- Ability to work independently and handle diverse caseloads.
