A Day in the Life in a CMHT team:

As a CPN specialising in adults delivering NHS community mental health services. Your responsibilities will include:

Working in a care coordination role plays a vital part in managing and delivering comprehensive mental health care to individuals within the community. The primary focus is on ensuring seamless integration of services and providing continuous support to patients.

Candidates should have previous community experience where assessments and caseload holding is required. Excellent assessment and communications skills are required. The post covers the geographical areas of South London and is most suited to someone who lives in London and is able to drive.

Requirements and qualifications required for this role:

  • Current and valid registration with the NMC.
  • Proven experience within a CMHT Team or community experience.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Commitment to providing high-quality patient care.
  • Members of staff should be a driver and have adults community experience.
