Job OverviewWe are looking for a motivated and skilled Band 6 Occupational Therapist to join the community health team. This role involves providing holistic, patient-centered care in various community settings, supporting individuals with complex rehabilitation needs to achieve independence and improve their quality of life.
Key Responsibilities- Conduct assessments in home or community settings, developing personalized care and rehabilitation plans.
- Deliver high-quality interventions to enhance patients' mobility, daily living skills, and mental well-being.
- Collaborate with multidisciplinary teams, including physiotherapists, nurses, and social care professionals, to provide comprehensive care.
- Educate patients, families, and caregivers on rehabilitation techniques, equipment use, and safety practices.
- Contribute to discharge planning, ensuring patients transition safely back to their home environment with proper support.
- HCPC registration as an Occupational Therapist.
- Experience in community healthcare or similar clinical settings.
- Strong assessment, planning, and rehabilitation skills with a patient-centered approach.
- Excellent communication and teamwork abilities, particularly in multi-agency collaboration.
- Ability to work independently and adapt to varied community environments.
