Job Overview

We are looking for a motivated and skilled Band 6 Occupational Therapist to join the community health team. This role involves providing holistic, patient-centered care in various community settings, supporting individuals with complex rehabilitation needs to achieve independence and improve their quality of life.

Key Responsibilities

Conduct assessments in home or community settings, developing personalized care and rehabilitation plans.
Deliver high-quality interventions to enhance patients' mobility, daily living skills, and mental well-being.
Collaborate with multidisciplinary teams, including physiotherapists, nurses, and social care professionals, to provide comprehensive care.
Educate patients, families, and caregivers on rehabilitation techniques, equipment use, and safety practices.
Contribute to discharge planning, ensuring patients transition safely back to their home environment with proper support.

Requirements

HCPC registration as an Occupational Therapist.
Experience in community healthcare or similar clinical settings.
Strong assessment, planning, and rehabilitation skills with a patient-centered approach.
Excellent communication and teamwork abilities, particularly in multi-agency collaboration.
Ability to work independently and adapt to varied community environments.
