Job Overview:We are looking for a motivated Band 6 Occupational Therapist to join the Acute and Frailty team at St Helier Hospital. The role focuses on delivering high-quality assessments, interventions, and discharge planning, ensuring patient independence and optimal hospital flow.

Key Responsibilities:

  • Conduct assessments and design care plans to enhance patient independence.
  • Manage complex mental health needs and coordinate effective discharge planning.
  • Work collaboratively with the Acute OT team to optimize patient care pathways.
  • Provide tailored interventions for frail patients in an acute care setting.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • At least 6 months of NHS experience in Acute OT/Frailty OT roles.
  • Expertise in handling complex mental health cases and discharge processes.
  • Strong teamwork and communication skills within a multidisciplinary environment.
