Job Overview:We are seeking an experienced Band 6 Occupational Therapist to join the community team in Marlow and Thame. This role emphasizes elderly care, conducting community visits, and delivering patient-centered therapy to enhance quality of life and promote independence.

Key Responsibilities:

Manage a community-based caseload focused on elderly care and rehabilitation.
Conduct home assessments and implement interventions to improve safety and independence.
Collaborate with multidisciplinary teams to create and execute comprehensive care plans.
Provide advice and support to patients, families, and carers to achieve therapy goals.
Maintain accurate and thorough documentation in line with trust policies.

Key Requirements:

HCPC registration as an Occupational Therapist.
Proven NHS experience in community and elderly care settings.
Knowledge of community rehabilitation practices and home safety adaptations.
Strong communication, organizational, and teamwork skills.
Access to a vehicle and a valid driving license for community visits.
