Job Overview:We are seeking an experienced and dynamic Band 6 Occupational Therapist to join the Specialty Rehab Wards at Chelsea and Westminster Hospital. This role involves working across general medical and elderly/frailty wards, focusing on rehabilitation and discharge planning to support patient recovery and independence.

Key Responsibilities:

• Provide specialized occupational therapy within general medical and elderly/frailty wards.
• Assess, plan, and implement individualized rehabilitation and discharge plans.
• Work flexibly across wards, managing rehab for patients under pressure and autonomously.
• Collaborate with multidisciplinary teams to ensure holistic and effective patient care.
• Supervise junior staff and contribute to MDT decision-making where applicable.

Requirements:

• HCPC registration.
• Proven experience in discharge planning and medical rehabilitation.
• Strong ability to work independently and manage a diverse caseload.
• Experience supervising junior staff.
• Familiarity with working in an MDT setting.
