As part of our community neuro team, you will deliver person-centred care, supporting patients in their homes or community settings to help them regain independence and improve their quality of life. The role is Monday to Friday 8.30-16.30 / Monday -Sunday 08.00-20.00.
Key Responsibilities:
Patient facing therapy assessments / risk assessments, clinical reasoning, intervention planning, therapy goal setting, discharge planning, equipment prescriptions and review.
Professional registration.
Experience with urgent/ rapid response, community working, use of EPIC, lone working
Qualifications and Experience:
Degree in Occupational Therapy and current registration with the HCPC (Health and Care Professions Council).
Proven experience in neurorehabilitation and working with patients with neurological conditions is essential.
Experience in community-based care is highly desirable.
Strong communication and organizational skills, with the ability to work independently and manage a caseload effectively.
