This exciting role will primarily be based in South West London, with occasional work across two boroughs. You'll be working with older adults and younger adults with physical disabilities, supporting their independence and well being through a strengths-based approach.

Key Responsibilities:

  • Specialist Assessments: Conduct detailed functional assessments, identifying care and support needs for service users with complex disabilities.
  • Strength-Based Practice: Focus on empowering service users to build on their strengths, promoting autonomy and resilience.
  • Leadership and Supervision: Provide leadership, supervision, and case management to OT and social care staff, offering guidance and expert advice on complex cases.
  • Moving and Handling Risk Assessments: Perform complex risk assessments to ensure safe and effective care delivery.
  • DFG and Adaptations: Assess and recommend both minor and major home adaptations, supporting the DFG process.
  • Operational Excellence: Support the day-to-day operational needs of the team while maintaining a focus on delivering high-quality services.
