Job Overview:We are seeking an experienced and self-motivated Band 6 Community Physiotherapist to join our dynamic team. This role involves delivering physiotherapy services to patients in community settings, focusing on enhancing mobility, independence, and quality of life. The ideal candidate will have a broad clinical background with experience in orthopaedics, care of the elderly, respiratory, and musculoskeletal outpatients.
Key Responsibilities:
- Conduct comprehensive assessments and provide tailored physiotherapy interventions in the community.
- Develop individualized care plans to address patients' physical and functional needs.
- Manage a caseload of patients with diverse conditions, including orthopaedic and elderly care needs.
- Provide respiratory physiotherapy support and rehabilitation for musculoskeletal outpatients.
- Liaise with multidisciplinary teams, families, and carers to ensure holistic patient care.
- Advise on and provide mobility aids and specialist equipment to promote independence.
- Deliver education to patients and families on managing long-term conditions.
Requirements:
- Degree or equivalent in Physiotherapy and current HCPC registration.
- Minimum 6 months of NHS experience, including community physiotherapy.
- Proven experience in orthopaedics, care of the elderly, respiratory, and musculoskeletal outpatients.
- Ability to work independently in a community setting and collaborate effectively with multidisciplinary teams.
- Excellent communication, organizational, and time-management skills.
- Full UK driving license and access to a vehicle is desirable.
