Westminster

Our Local Authority client are seeking an Occupational Therapist to join their Reablement Team in Westminster for a 3 month initial contract.

The CIS is an integrated health and social care service designed to provide urgent support to individuals with disabilities or those recovering from injury, helping them build the confidence and skills needed to stay safe and independent at home.

Key responsibilities:

  • Conduct functional assessments and develop tailored reablement programs for service users referred to CIS.
  • Set and review goals, coordinating support to ensure customers can safely manage daily activities with maximum independence.
  • Complete discharge assessments to identify ongoing needs and eligibility for continued support in line with the Care Act 2014.

Requirements:

  • Must have at least Diploma/BSc in Occupational Therapy or equivalent and have HCPC registration.
  • Knowledge and understanding of care coordination and the Trusted Assessor role within a social care/health setting.
  • Basic understanding of conditions leading to disability and the services available to support those affected.
