Westminster City Council is looking for a HCPC-registered Occupational Therapy Care Manager (OTCM) to join the Community Independence Service (CIS) Reablement Team. This is a frontline role supporting residents to regain independence following hospital discharge or health crises. The successful candidate will work as part of a multidisciplinary team, including Occupational Therapists, Independent Living Assessors (ILAs), and Social Workers, and will collaborate with health professionals and Reablement care providers. The client offers a full-time contract with an opportunity to work from home, a fair and manageable caseload and further training in your portfolio. The vacancy will be for an initial 3 - 6 months with potential for extension.

Main Duties for the Occupational Therapist includes.

  • Assessment of service users and carers with complex needs in relation to home environment
  • Deliver skilled Occupational Therapy interventions to residents in their homes.
  • Manage cases for up to six weeks with a goal-focused reablement plan.
  • Conduct holistic assessments to identify clinical and personal needs.
  • Develop and implement intervention plans, including complex movement and handling assessments.
  • Order and provide adaptive equipment to support independent living.
  • Work collaboratively with a multidisciplinary team to ensure effective service delivery. Maintain accurate case records using the Mosaic system.
  • An awareness of the importance of safeguarding vulnerable people, plus an understanding of \n the importance of dignity & respect when dealing with people.

To Be Successful.

  • Experience in adult social care or community settings.
  • Experience in complex movement and handling assessments and care planning (training and supervision available). Strong case management skills, with experience in goal setting and strength-based interventions.
  • Ability to work effectively in a fast-paced, multidisciplinary team.
  • IT proficiency, with experience in Mosaic preferred.
  • HCPC Registration
  • Enhanced DBS/CRB (we can obtain for you)
  • Eligibility to work in the UK
  • Occupational Therapist Qualification
