Job Description:
We are seeking a highly motivated and experienced Band 6 Physiotherapist to join our Allied Health Professionals team. This role focuses on delivering high-quality physiotherapy care to housebound patients in the community, with a particular emphasis on fall prevention and management.

Key Responsibilities:

  • Conduct community visits to assess and treat housebound patients, delivering tailored physiotherapy interventions.
  • Triage patients for the Falls Practitioner team, prioritizing urgent or high-risk cases for referral to the BHR Falls Service.
  • Develop and implement individualised care plans to reduce the risk of falls and improve mobility and independence.
  • Provide education and support to patients, families, and caregivers on fall prevention strategies.
  • Collaborate with multidisciplinary teams to ensure a holistic approach to patient care.
  • Maintain accurate documentation in line with professional and organisational standards.
  • Participate in service development and quality improvement initiatives within the community and falls service.

Requirements:

  • Experience working with elderly patients, in community settings, and managing falls cases.
  • Strong clinical assessment and decision-making skills.
  • Excellent communication and teamwork abilities.
  • A valid UK driving licence and access to a car, as the role requires travel within the community.
