Join our team as a Social Services Assessor and play a key role in supporting adults in the community to live more independently. In this role, you will:

Perform holistic assessments of clients within a community setting
-
Identify and provide appropriate support interventions
-
Recommend and arrange equipment or home adaptations to improve quality of life
-
Collaborate closely with a multi-disciplinary team to ensure a seamless, person-centred care experience
-
Maintain accurate and up-to-date records
To be successful in this role, you will need:

A current and valid HCPC registration
-
Proven experience in adult social care or community-based assessment
-
Strong communication and interpersonal skills
-
Ability to work both independently and as part of a multidisciplinary team

A genuine commitment to improving the lives of vulnerable adults
This flexible locum position requires availability from Monday to Friday with some potential for hybrid working depending on the service’s needs. The contract is ongoing, with a strong possibility of extension based on performance and service demand.
