Personal Advisor - Leaving Care Role

We are seeking a compassionate and motivated Personal Advisor to support young people transitioning from care into independent living. In this vital role, you will build strong, trusting relationships with care leavers, helping them navigate challenges and achieve their life goals.

You will regularly visit young people where they are living, ensuring their well-being and addressing any concerns. Collaborating with foster carers, keyworkers, and significant others, you will ensure they feel settled, supported, and focused on their future aspirations. Central to this role is guiding young people through the pathway planning process, updating their plans in partnership with them, and helping to implement their goals.

Your responsibilities will include supporting their education, helping them develop essential life skills, and providing advice on career opportunities, training, and housing. You will work to ensure they have access to health and dental care, emotional and psychological support, and resources to maintain overall well-being. Through regular review meetings, you will continue to adapt their plans to meet their evolving needs, empowering them to build independent, fulfilling lives.

The role also involves encouraging young people to engage with their community, participate in activities, and access volunteering opportunities, helping them feel a sense of belonging. You will support them in managing a tenancy and transitioning into independent housing when the time is right.

This position holds a caseload of care leavers, ensuring they receive consistent, reliable support. You will provide services up to the age of 25, based on the individual needs and requests of each care leaver.
