To be responsible for safeguarding and improving the well-being of a group of key service users to enable them to increase independence, maintain their accommodation and enjoy a good quality of lifeTo visit and support service users within their homes and the communityTo enable the service users to access medical services and the local community mainstream servicesTo ensure that allocated service users have a needs & risk assessment and SMART support plan; which are reviewed at agreed frequenciesTo liaise effectively with other agencies involved in the support of service users where appropriateTo encourage users to have on-going engagement with existing networks including faith-based groups and informal sources of supportTo provide information and advice on benefits, housing and welfare issues to service usersTo facilitate access to self-management programmesTo build strong links with community organisations and work with them to support wider engagement with service usersTo work with universal services to ensure users have access to appropriate community based facilities, promoting community cohesionTo assist the Outreach Team Leader to develop strong working partnerships with local providers particularly within universal and mental health service settingsTo support users in accessing mental health resources, enabling access to opportunities for recovery and rehabilitation and community capacity buildingTo assist the Outreach Team Leader to develop effective working partnerships with Faith Groups, Leisure Providers, Colleges, Employers and Employment OrganisationsTo promote positive attitudes in public services and in the local community by providing information and talks to raise awareness of mental health issuesTo work with the Outreach Team Leader to devise a strategic plan to engage difficult to engage and hidden users, developing meaningful services that are based around genuine needs and goalsEnsure service users are consulted in relevant aspects of service management. Implement and /or take part in effective consultation, participation and communication mechanisms with individual service users and service user groupsTo attend any relevant forums and meetingsSkillsKnowledge of welfare benefits entitlements appropriate to the user groupKnowledge and understanding of safeguarding issues and ability to address them appropriatelyKnowledge of key working, person-centred support planning systems and the recovery approachExperience of delivering support to people within a community/outreach settingAbility to support service users to achieve their goals & aspirationsAn understanding of equality and diversity and how it applies to the job role and the importance of promoting service users’ rights, privacy dignity and choiceWork experienceExperience in support worker, safeguarding, substance and drug misuse advocacy, referral, and resettlement is ideal for this positionHours of Work:39 hours per week on average
