The London Borough of Richmond and Wandsworth are looking for a Social Care Assessor to join their Preparing for Adulthood Service, based in Wandsworth
As a Social Care Assessor you will:

  • Assess and support plan applying the principle of good conversation, strengths-based practice, motivational interviewing
  • Assess for eligible care needs in accordance with the Care Act (2014) and input urgent care packages if required
  • Signpost people to other services that will be beneficial for them
  • Complete review preventative support that have been put in place
  • Provide a holistic, personalized, strength-based service to residents between the ages of 18-25 who have physical, learning and sensory disabilities as well as mental health social care needs

Requirements

  • Social Care Qualification
  • Strong working knowledge of preventative services, enablement and link people in with these promoting a "try it and see" ethos
  • Have an understanding of the Care Act 2014 and the roles and responsibilities of a Social Services Department including safeguarding
  • Ability to manage your own caseload
