Rate: 22 Umbrella P/H

We are seeking a motivated and compassionate Social Care Officer to join our dynamic team in Newham. In this vital role, you will provide high-quality, responsive, and safe assessment, support planning, and review services to adults and their families/carers. This is an opportunity to make a meaningful difference by empowering individuals, promoting independence, and ensuring positive outcomes.

Key Responsibilities

  • Conduct assessments, support planning, and reviews to meet the needs of adult customers and their carers, ensuring compliance with national and local standards.
  • Deliver personalized services that promote independence, choice, and risk management.
  • Assist customers and carers with supported self-assessments, identifying innovative solutions to meet their needs.
  • Manage an allocated caseload effectively, providing a professional social care service.
  • Contribute to safeguarding adults by assessing risks and developing multi-agency safeguarding plans.
  • Act as a lead in meetings with customers, their families, and external agencies to ensure inclusive and effective support.
  • Maintain up-to-date, accurate, and clear records in line with statutory requirements.
  • Collaborate with internal and external partners to strengthen support networks and improve service delivery.

What We're Looking For

  • Experience working in a social care or related setting.
  • A commitment to delivering high standards of practice, including person-centered and outcome-focused services.
  • Strong knowledge of safeguarding, risk management, and the Mental Capacity Act 2005.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to manage a varied caseload and work collaboratively within a multidisciplinary team.
