Key Responsibilities
- Coordinate care for residents, focusing on their strengths and community networks.
- Conduct quality conversations, triage, initial assessments, support planning, and case coordination.
- Collaborate with social workers, occupational therapists, and support staff to ensure seamless service delivery.
- Work within legal frameworks, safeguarding principles, and strengths-based practices to optimize outcomes.
- Promote independence by connecting residents with voluntary and community groups.
About YouWe are looking for individuals with:
- A QCF Level 3 qualification in health and social care (or willingness to achieve it).
- Experience in social care, health, or the voluntary sector.
- Strong knowledge of legislation and guidance related to adult care.
- Excellent communication, negotiation, and advocacy skills.
- A proactive, solution-focused approach to challenges.
