The Role: Adult Case Manager

Location: Islington

Pay: £25 P/H

Key Responsibilities

  • Coordinate care for residents, focusing on their strengths and community networks.
  • Conduct quality conversations, triage, initial assessments, support planning, and case coordination.
  • Collaborate with social workers, occupational therapists, and support staff to ensure seamless service delivery.
  • Work within legal frameworks, safeguarding principles, and strengths-based practices to optimize outcomes.
  • Promote independence by connecting residents with voluntary and community groups.

About YouWe are looking for individuals with:

  • A QCF Level 3 qualification in health and social care (or willingness to achieve it).
  • Experience in social care, health, or the voluntary sector.
  • Strong knowledge of legislation and guidance related to adult care.
  • Excellent communication, negotiation, and advocacy skills.
  • A proactive, solution-focused approach to challenges.
