Assistant Team Manager/Practice Manager, Older People and Physical Disabilities TeamPay rate - £40 per hourVitalis are looking to recruit an Assistant Team Manager/Practice Manager to work in an Older People and Physical Disabilities Team in Southwark.Experience with various complex caseloads in OPPD Prior experience using IT Databases e.g. Mosaic/Liquid Logic, excel, word Safeguarding investigation experience.2 years’ experience post ASYEThe Case Management team undertake longer-term (6 weeks or more), complex social work interventions, working with older adults and physically disabled people, including those with concurrent mental health and substance misuse needs. The team's responsibilities encompass complex mental capacity work, including Court of Protection applications when necessary. They also conduct Safeguarding enquiries, assessments, and unscheduled reviews under the Care Act 2014.
