The Deputy Team Manager will be responsible for the day-to-day operational oversight of social workers and their work products. This includes providing social work practice guidance and management oversight relating to case progression and management of - often complex - discharges, complaints and disputes, safeguarding and mental capacity. The post holder will take an active part in networking with NHS trust colleagues and form part of the management team overseeing team performance and attending regular discharge review meetings, updating the acute trust about discharge plans and responding to system escalations as pressures on acute trusts increase. A proven track-record of multidisciplinary working/close partnership working with other local authority colleagues/GPs/NHS and a sound understanding of ordinary residence and current out-of-hospital pathways is essential for this role. Good working knowledge of the Continuing Healthcare process and the relevant framework is essential for this role.
