About the Role:We are seeking a dedicated and compassionate Supported Pathways Officer to join our dynamic Resident Services team. In this role, you will work closely with residents to assist them in securing their next-stage housing, providing tailored resettlement support, and linking them to necessary services.

Your caseload will include approximately 10 clients, and your primary focus will be to help them maintain their current accommodation, develop life skills, and improve their physical and mental wellbeing. With an understanding of Adverse Childhood Experiences and Trauma-Informed Care, you will empower residents to break the cycle of homelessness and achieve stability.

Key Responsibilities:

  • Provide holistic, client-centered key-working, risk assessments, and support plans.
  • Assist residents in developing life skills and engaging with meaningful activities.
  • Coordinate with external services such as health agencies, social services, and community organizations.
  • Ensure residents' safety, health, and wellbeing through risk management and health checks.
  • Advocate for residents by accompanying them to appointments and providing practical support.
  • Monitor rent and service charge accounts, assisting residents in managing arrears and maximizing income.
  • Support the upkeep and management of hostel facilities, ensuring compliance with health and safety standards.
